Membership Application 

The Alzheimer Society of Washington has conservative fund-raising goals - we want to continue to reach persons, families and those who affect the lives and care for persons with dementia.

The Alzheimer Society deeply appreciates support from those who are able to contribute but the Alzheimer Society does not want to become known as a "fund raising" organization. However, we do rely on the generosity of our supporters.

For your contribution please choose the level that is appropriate for you. Fill out the form below:

Membership is for the current calendar year.

Individuals/Organizations will receive:

  • General schedule of Alzheimer Society events
  • Newsletter
  • Reduced fee for individuals attending Alzheimer Society training sessions
  • Use of library resources at no charge ($3.00 postage & handling for each item mailed)
  • Use of videos at no charge ($20 refundable deposit for each video requested)
  • Information about participation in dementia care advocacy

* Individuals who have low incomes may request membership at a reduced rate.

** Membership granted to a care facility will state "Membership in the Alzheimer Society of Washington does not signify the Society's endorsement of the facility's philosophy of care."
